Ketamine acts primarily on the receptors in the brain, which play a central role in mood, memory, and perception. Ketamine blocks their normal function by preventing the flow of the signals they need to operate.
Because ketamine works through the NMDA receptor pathways, stopping ketamine is mostly a psychological experience, unlike alcohol and benzodiazepine withdrawal which can produce seizures and potentially life-threatening physical symptoms.Ketamine use produces an antidepressant-like effect on the brain. When you’re dependent on ketamine and quit, depression and emotional instability are among the most common withdrawal effects.

Ketamine withdrawal symptoms – what to expect
How severe withdrawal symptoms are depends on how long and how heavily you’ve been using ketamine. But certain patterns are consistent in most cases.
The psychological symptoms of ketamine withdrawal are typically the most significant. Disruption to the receptor system that it acts on, means stopping use might affect mood, memory, and reward processing.

Psychological withdrawal symptoms may include:
- Intense cravings
- Anxiety and low mood
- Mood swings and agitation
- Difficulty sleeping

- In severe cases, psychosis including hallucinations
Physical withdrawal effects are generally less severe than those seen with alcohol or opioid withdrawal, but they still need monitoring.

Ketamine withdrawal timeline
Ketamine withdrawal is generally thought to move through three phases, though individual experience varies and this is based on general information available, rather than primary research:
Phase 1 (0–24 hours):
In the first 24 hours after stopping, acute symptoms begin. These include anxiety, restlessness, and emotional instability. Cravings begin early and can be intense. Heart rate may increase, and sweating is also common.
Phase 2 (days 1–14):
From days one to 14, symptoms reach their peak, with the psychological effects remaining intense throughout. Sleep becomes difficult, cognitive fog gets worse, and mood can be very unstable. Physical symptoms, including elevated blood pressure, nausea, and fatigue, begin to ease toward the end of this period.
Phase 3 (weeks to months):
From around day 15 onwards, most physical symptoms have usually eased, but psychological symptoms often continue for weeks or months. Depression, cravings, and emotional sensitivity are common during this period.

From ketamine detox to recovery. A connected programme
Ketamine detox at Smarmore Castle is the first phase of treatment. As soon as you’re ready, individual therapy begins alongside detox. Group therapy and Cognitive Behavioural Therapy (CBT) run throughout your stay.
Your treatment programme is planned, delivered, and adjusted as needed, with input from both your therapy and medical teams. The programme is continuous from the first day to the last, with no point where one team hands your care over to another.
Once your stay at Smarmore Castle is done, we provide continuing care. This helps with the transition back home and the important post-treatment months when relapse risk is highest.

Psychosis, including hallucinations and severe perceptual disturbances, have been known to occur during ketamine withdrawal, but is often only for those that have used it for a prolonged period. This is likely in the first 72 hours, and is part of why medical supervision during the first days is needed.

Some people need ongoing urological assessment and supportive care to manage the symptoms of this.
